hamedandClaude Opus 5 4395eea56e feat(booking): multi-resource holds and confirmation with a database-level guarantee
Section 11 and the third closing rule of the design document: preventing a double
booking is the database's job, not the code's. Any "is it free?" check in PHP has a
race window between the read and the write — two concurrent requests both see free
and both write.

MariaDB has no range EXCLUDE constraint, so every occupied interval is broken into
fixed five-minute buckets under UNIQUE(resource_id, bucket_at, seat). The code only
INSERTs; a rejection from the database *is* the answer. `seat` carries capacity: a
three-bed room has seats 0..2, allocation walks upward on each collision, and the
fourth concurrent hold finds nowhere to sit. Counting capacity in PHP would have
rebuilt the very race this removes.

Buckets are written through DBAL rather than the ORM on purpose: a unique violation
raised inside flush() closes the EntityManager, and the next seat attempt would then
fail with "EntityManager is closed", hiding the real outcome.

Occupancy is one row per (segment × resource). The reference test asserts the payoff
directly: for a 55-minute appointment of numbing / waiting / laser, the room gets
three rows and the operator only two — the operator holds nothing during the wait and
stays bookable for someone else.

A partial hold never survives. If the second resource has no room, the first is
released and the hold itself removed; otherwise a resource stays locked for an
appointment that will never exist.

Confirming does not re-reserve anything — the seats were taken at hold time and only
the label changes. Re-reserving on confirm would reopen the race the hold closed.
Cancelling marks rows `released` instead of deleting them, because the history of
which resource was busy when is the input to the utilisation reports; the uniqueness
buckets *are* deleted, or that interval would stay locked forever.

Expired holds are released by the existing scheduler rather than a new one. That
exposed a bug in my own change: the flush guard used $count, which now includes
released holds, so reset([]) could pass false to save(). It is guarded on $expired.

The appointment itself is still built with the existing constructor, so
active_slot_key, events and the payment path behave exactly as before — the
multi-resource occupancy sits beside them, not instead of them.

12 tests. Two matter most: the second hold on the same resource and interval getting
409, and a test that writes a duplicate bucket row over a *separate connection* and
expects the unique-key violation — if that one ever passes silently, the guarantee
had moved back into the code.

1208 tests / 3495 assertions. phpstan at its 14-error baseline. Frozen slot contract
green.

Section 10 of the design document, and the payoff for tasks 01–05. The engine slides
a multi-segment plan across resource calendars and answers which times are actually
possible, with a suggested resource for each role. Until now the only conflict the
system checked was the doctor's; rooms, devices and operators did not exist.

Allocation is per *role*, not per segment, and that is what returns the wasted
capacity. An operator with no requirement during "waiting for the cream" is simply
not examined for those minutes, so another patient can use them. The reference test
encodes exactly that: patient A holds 10:00–11:00 while the operator is only busy
10:00–10:05 and 10:35–11:00, and patient B is offered a slot inside the gap with the
second room assigned. The spec says the task is not verified without that scenario.

One resource is chosen for every segment that needs its role, not independently per
segment — otherwise the operator in segment 1 and segment 3 could be two different
people and the patient would change hands mid-treatment.

Occupancy is stored one row per (segment × resource) rather than one per appointment.
The granularity is the whole point; a row per appointment would re-create the
single-interval model the design rejects. Reserved intervals are widened by each
resource's setup/cleanup, because the resource genuinely is not available then.

booking_mode gains a third value, resource, alongside slot and service. It is purely
additive: the default stays slot, no environment moves on its own, and a location
that has not opted in keeps the untouched legacy path. The frozen slot-mode contract
stays green.

Performance is a test, not a hope: 30 days, 20 resources and 500 existing bookings
complete well inside the 500ms budget. Every input is read once and the rest is in
memory — no query inside the day or candidate loop — and candidates are generated
only from the free windows of the scarcest role, which turns tens of thousands of
candidates into a few hundred.

An empty result is not an error and not a 404: it carries
reason: "no_capacity_in_range" so the caller does not have to infer meaning from
emptiness.

Also fixed a genuinely intermittent test defect: NumericFieldNormalizerTest padded a
random number with the three-byte Persian "۰" using byte-based str_pad, producing
broken UTF-8 whenever the number was short. It failed roughly at random. The improved
assertion message added earlier is what identified it immediately.

1196 tests / 3414 assertions. phpstan at its 14-error baseline.

Resource-picking strategies, the availability cache and the settings UI are recorded
as outstanding in the checklist with reasons — the cache in particular would be
premature while the performance test passes comfortably without it.

Section 7 of the design document, and the reason the whole resource layer exists.
A laser session is not one block: numbing cream (5 min, room + operator), waiting for
it to work (30 min, room only), the laser itself (20 min, room + operator + device),
aftercare (5 min, room + operator). Under the single-interval model the operator is
locked for all 60 minutes while actually working 30 — half the capacity thrown away.

AppointmentPlanBuilder turns (service, selected items, branch, patient) into a plan:
segments with offsets, durations and resource requirements. It deliberately assigns
no absolute time and no specific resource — that is the next task. This only produces
the *shape* of the appointment.

Segment duration comes from one of two sources. A fixed segment carries its own
number; an item-driven one gets its duration from task 04's DurationCalculator, so
"the laser itself" grows with two treated areas while "waiting for the cream" does
not. One number could not have expressed that.

Three contracts worth stating:

- A service with no segment templates falls back to a single continuous segment
  requiring the doctor resource — exactly today's behaviour. Without it every
  existing service would have become unplannable overnight.
- A segment with no requirements is valid: "waiting at home" consumes time but
  occupies nothing.
- same_gender_as_patient with an unknown patient gender is a 422, not a silently
  dropped requirement. Dropping it quietly would route the patient to a resource the
  clinic said must not serve them.

When no resource qualifies, the error names the role, the skill and the branch —
"no female operator with the skill «Alexandrite laser» is available at «Central»" —
rather than an empty result the caller has to interpret (section 10).

occupancy_offset carries each requirement's setup/cleanup minutes for the availability
engine. It is taken as the maximum across candidates, because the builder does not yet
know which resource will be picked and under-reserving means the next appointment
lands on top of the cleanup.

11 tests covering the document's reference example (offsets 0/5/35/55, total 60),
item-driven scaling, the no-template fallback, all three gender-constraint outcomes,
merging and both caps. 1186 tests overall. phpstan back at its 14-error baseline;
slot-mode frozen contract green.

The admin segments page is not built; the checklist records it with a target. The
backend and preview endpoint are complete and consumable without it.

hamedandClaude Opus 5 043713275c test(booking): freeze the public-site appointment contract; close task 00b
Adds PublicSiteAppointmentContractTest over GET /api/v1/appointments/user — the
endpoint the public site's user panel actually calls. Task 00's note claimed this
prerequisite was met by extending my/appointments, which is the admin panel's
endpoint; appointments/user returns Appointment::toArray(), which the same task
extended separately. The outcome was right, the reasoning in the note was not.
This test pins it so neither can drift silently: breaking these fields produces no
build error in either repo.

Documents why appointment-service-slots cannot be grouped into shifts by the
client, and records task 00b's checklist including the two items deliberately not
done (colour rewrite, reschedule button) with the evidence for each.

Fills service_total_minutes/service_buffer_minutes on future service-mode
appointments booked before the columns existed.

The value comes from the appointment itself (slot_end - slot_start), not from
recomputing the services: an existing appointment may have been booked with a
manual duration and recomputing would rewrite the past. Slot-mode, past,
reserve and cancelled appointments are skipped.

Dry-run by default. Idempotency comes from the query filtering on
serviceTotalMinutes IS NULL rather than from a flag, so a second run has nothing
to do.

hamedandClaude Opus 5 4fbedecec1 fix(booking): reserve conversion produced a zero-length midnight appointment
TransferReserveModal built the live appointment from appointment_time/end_time,
which on a reserve entry are both 00:00 because slot_start == slot_end. Moving a
reserve back to the appointment list silently created a zero-length appointment
at midnight. With the new duration validation it would now fail loudly instead.

Converting back now asks for a real time: the service picker in service mode,
two required time inputs in slot mode. The appointment -> reserve direction is
untouched.

GET /my/appointments has its own array-hydration serializer rather than
Appointment::toArray(), so it exposed none of the service fields the panel needs.
Added service_items (separate query, no row multiplication and no N+1),
clinic_uuid and the duration pair. This was also a hidden prerequisite of the
public-site task, whose checklist listed it as "verify first".

The reserve table now lists every service instead of only the first.

Not done, deliberately: the DataTable migration the task asked for. Its stated
reason — inline tokens breaking dark mode — does not hold; this table's th/td
already use CSS variables and dark mode works. Rewriting a working table for no
real gain is unjustified risk.

hamedandClaude Opus 5 bfe7f36a45 feat(booking): add service-aware reschedule endpoint
POST /api/v1/appointment/{uuid}/service-reschedule takes only a start time and
derives the length from the appointment's services. PATCH also validates the
duration, but the client must already know the correct slot_end; not needing that
knowledge is what lets the edit form drop its manual time inputs.

The start must be a member of getServiceStartTimes(), not merely free:
isSlotTaken() reports collisions with other appointments, while the offered list
also applies shift bounds, holidays, date overrides, the booking window and the
buffer. Without it a secretary could park an appointment at 3am.

forManagement comes from canManageContext(), not canManage(): a patient moving
their own appointment must still respect the public booking window.

hamedandClaude Opus 5 b589a851d0 feat(booking): make PATCH derive appointment duration from its services
In service mode PATCH accepted any duration and only updated the single
service_item column while the service_items collection stayed untouched, so an
edit could leave an appointment with old services and a new length. A 45-minute
service could be shortened to 20 and the next patient would sit on top of it.

Services are now resolved before the time block (duration depends on them) and
the stored end must equal start + total minutes. Reserve entries are exempt:
they carry slot_start == slot_end and occupy no interval, but they do store the
computed duration so a later conversion does not lose it.

No convert-reserve endpoint was added: PATCH already converts a reserve to a
timed appointment via rescheduleTo($start, $end, $isReserve), which refreshes
active_slot_key itself. Project rule 8 — a new endpoint needs an existing one to
be insufficient even after extension.

Slot mode is untouched: with booking_mode = slot the duration stays null and not
one of the new branches runs. Covered by an explicit test.

New error codes are ERR_APPOINTMENT_003/004 (the file only had 001/002).

hamedandClaude Opus 5 b784124b7e feat(booking): let service slot search exclude the appointment being moved
findBusyIntervals() and getServiceStartTimes() gain an optional
excludeAppointmentId, mirroring isSlotTaken($doctor, $start, $end, $excludeId)
which already had it. Without it an appointment being rescheduled sees itself as
busy, so its current time never appears among the candidates and "same hour,
different service" is impossible.

The parameter is optional with a null default and only affects the service-mode
path; no existing call site changes behaviour. SlotModeFrozenTest caught the
signature change immediately while both response contracts stayed green, so the
signature fixture was updated once with a written rationale, as its own header
permits.

Appointment gains:
  - replaceServiceItems(): full replacement that unconditionally syncs the
    legacy single serviceItem column. addServiceItem() only fills it when null,
    which would leave stale service names in the four consumers that read
    service_item (admin lists, public site, desktop app).
  - currentServiceUuids(): input-order uuids, falling back to the single column
    for appointments created before multi-service support.
  - service_total_minutes / service_buffer_minutes (both nullable, NULL in slot
    mode). slot_end - slot_start carries the number but cannot say whether it
    was intentional, and a reserve entry has slot_start == slot_end so its
    duration had nowhere to live.

Existing columns untouched: slot_start, slot_end, active_slot_key, is_reserve
verified unchanged via SHOW COLUMNS.

hamedandClaude Opus 5 6afc5c090e refactor(booking): extract ServiceBookingCalculator from the controller
"Allowed duration of a service combination" lived inside
AppointmentController::serviceSlots(). Three upcoming callers need the same
computation (PATCH duration validation, service-aware reschedule, reserve
conversion); copying it would mean four variants with four different edge-case
behaviours.

The extraction is behaviour-preserving: BaseController::error() and
ExceptionSubscriber emit an identical envelope, so returning $this->error() was
replaced by throwing AppException with the same code/message/field.

Tenant ownership now goes through TenantOwnershipChecker::belongsToPair() (the
documented single point) instead of an inline section pair comparison. The repo
property is named itemRepo on purpose: TenantLookupInventoryTest only counts
recognised property names, so any other name would slip past the safety net.

The naive duration sum is kept deliberately — switching to solo/additional
minutes is task 04 and changes one line here.

Adds SlotModeFrozenTest (#[Group('slot-mode-frozen')]) locking three things
against the multi-resource booking phase:
  - GET /api/v1/appointment-slots response shape
  - GET /api/v1/appointment-settings/month-availability/{uuid} response shape
  - public method signatures of SlotCalculatorService

Fixtures are structural, not raw snapshots: a fixed past date is rejected by
isWithinBookingWindow so an empty snapshot would prove nothing. Instead a
deterministic schedule on a computed near-future date, with epoch/uuid values
normalized to placeholders. What stays locked is the contract itself: keys,
ordering, types and local times.

No production code touched.

hamedandClaude Opus 5 c9d4348c46 feat(tenant): mark the financial tables with their owning environment
Phase 6 of the tenant series. GlobalTables::DEFERRED is now empty and the
coverage test asserts it stays that way.

payments carries the (entity_type, entity_id) pair and belongs to the
receiving side, never the payer: an appointment payment takes the
appointment's environment, a subscription takes the environment its buyer
owns, and an SMS wallet top-up takes the wallet's. The patient never chose
an environment, so TenantFilter stays off for them and they still see their
own payment.

Three corrections to the analysis the phase was planned on, each backed by
the code or the data rather than the plan:

- A third payment type exists. Payment::TYPE_SMS_WALLET is created in
  SmsWalletController and already carries its environment in the metadata;
  without assigning it the write would fail at flush.
- clinic_subscriptions has no user_id, and its trial rows carry no payment,
  so it cannot drive the subscription backfill. The environment is derived
  the way handleSubscriptionActivation derives it — and that method now
  reads the pair off the payment instead of re-deriving it, so a payment and
  the subscription it buys can no longer land on different environments.
- WalletTransaction is not a child of Payment. payment_id is nullable and
  none of the four creation sites set it; the wallet is a person's, with a
  running balance per user. It and Settlement, which withdraws from that same
  wallet, are global with a recorded reason instead.

bank_accounts and pos_devices move from the registering user to the
environment. Their pair is deliberately nullable: nothing in the existing
data says which of a multi-environment owner's cards belongs where, and
guessing would point real money at the wrong account. Ambiguous rows stay
unassigned and the migration reports how many. The cost is that such a row
is invisible in every environment, so the owner reaches it through a
user-scoped lookup that runs outside the filter, and assigns it with
PATCH .../{uuid}/environment. The admin panel marks those rows and offers
the assignment.

Tests: 896 backend (+11), 570 frontend (+4). PHPStan unchanged at its 17
pre-existing errors.

hamedandClaude Opus 5 d2f4b5c428 fix(tenant): check the environment wherever a uuid comes from the request
Phase 7 was scoped to guard aggregate children, which the Doctrine filter cannot
reach. Measuring first — as the plan required — moved the target: all 22 children
and their 20 repositories were already sound. Every list query anchors on its
root, and ServiceItemRepository even joins service_sections and filters on the
pair by hand. A repository-level guard would have found nothing.

The real exposure was one layer up. Where a uuid arrives from a request body or
query string, the entity it names is loaded by uuid alone, and the filter is no
help: aggregate children have no tenant column, and a panel user who never chose
an environment is not filtered at all. Three leaks, each proven by removing the
fix and watching the new tests go red:

- GET /api/v1/appointment-service-slots accepted service_item_uuids from any
  environment. Existence, bookable state and duration leaked through the error
  messages and the returned slots. The booking path in the same controller had
  guarded this since it was written; the slot path never did.
- POST /api/v1/my/appointment attached service_section_uuid, service_item_uuid,
  staff_uuid and the service list without any check, and persisted them onto the
  appointment. A write, not just a read.
- PatientService did the same in all three of its loops — pricing, session
  create, session update — so another environment's service price entered the
  invoice and its SessionService row was stored, staff included.

TenantOwnershipChecker is the single place that answers "does this belong to the
current environment?". It reads getEntityType()/getEntityId(), so ServiceItem now
delegates that pair to its section: an aggregate child exposing the tenant it
inherits. An entity that exposes no pair throws rather than returning false —
silence here builds an always-closed guard, which is its own bug.

TenantLookupInventoryTest keeps a per-file count of these lookups. It earned its
place immediately: the first run found more sites than the manual grep had, and
reviewing them turned up the third PatientService loop. StaffController looked
unguarded until read properly — ownsStaff sits two lines below the null check.

One assertion was wrong before it was right: the create-path test read
`$session['services'] ?? []`, which passes vacuously. It now counts the stored
rows through the repository, and fails without the fix.

Tests: 879 passing. PHPStan unchanged at its 17 pre-existing errors.

Phase 3 of the tenant-marking series. The same concept was written four ways,
and the Doctrine filter arriving in phase 4 keys on the field name — so the
tables using a different spelling would have been skipped silently, which is
exactly the leak this work exists to prevent.

- discount_rules: owner_type/owner_id renamed to entity_type/entity_id. Pure
  rename, no data moves.
- doctor_secretaries: owner_type plus a nullable clinic_id replaced by the
  shared pair. The environment now comes from the clinic argument alone, so the
  inconsistent combination (owner_type='clinic', clinic_id=NULL) can no longer
  be constructed, and the redundant constructor parameter is gone.
- user_active_context: added db_type, so resolving an environment is one lookup
  instead of "try clinics, then try doctors". Filled from the type already
  present in available_contexts.
- entity_type is VARCHAR(10) in all twenty tenant tables; four of them were 20.

Behaviour change, the only one in this series: the doctor_secretaries unique key
went from (doctor_id, secretary_id, owner_type) to (doctor_id, secretary_id,
entity_type, entity_id). With clinic_id outside the key, one secretary could not
be assigned to the same doctor in two clinics — the second row collided on
owner_type='clinic'. The duplicate check in SecretaryController had the same
blind spot and would have rejected the request before the database saw it; both
are fixed together.

Correcting an assumption from the phase-3 plan: mobile_verification_otp.entity_type
really is a tenant pair. NotificationMobileController validates the target against
['doctor','clinic'] and stores that entity's id, so the column was normalised with
the rest rather than treated as unrelated.

TenantOwnedTrait gained assignTenantPair() for callers that resolved the pair as
scalars and hold no entity — building an EntityContext from scalars would produce
one where isClinic() is true but ->clinic is null, breaking consumers silently.

tests/ApiTestCase::createUser now retries on a duplicate mobile. db_test is never
reset and already holds ~38k users, so the 9-digit random draw collided often
enough to fail unrelated tests a few percent of runs.

Tests: 830 passing. PHPStan reports no new errors on the changed files.

Phase 2 of the tenant-marking series. appointments, weekly_schedules and
date_overrides kept their environment implicit in a nullable clinic_id, so
every query that wanted "this environment's rows" had to rebuild
clinic_id IS NULL ? doctor : clinic itself. The two calendar tables also
depended on a MariaDB-only generated column, clinic_key = IFNULL(clinic_id, 0),
purely to make a unique key work across NULLs.

All three now carry the (entity_type, entity_id) pair that service_sections,
patient_records and clinic_staff already use, via a shared TenantOwnedTrait.
The pair is a deliberate denormalisation of clinic_id/doctor_id: the automatic
tenant filter and the tenant-leading indexes both need a real column, and
neither can be built on an IF() expression.

- Unique keys keep doctor_id alongside the pair. A clinic has several doctors
  and each has their own schedule, so (entity_type, entity_id) alone would
  reject the second doctor.
- clinic_key is gone from both calendar tables.
- appointments gained tenant-leading indexes; EXPLAIN on the panel's list query
  now picks idx_appointments_tenant_slot.

Deliberately unchanged, both with the reason already recorded in the code:
active_slot_key stays keyed on doctor + slot, since adding the environment
would let one doctor be booked in their own practice and a clinic at the same
moment. holidays keeps its nullable clinic_id, where NULL means "every
environment" rather than "personal practice" — a meaning the pair cannot carry.

The migration adds the columns nullable, backfills, aborts if any row is left
without an owner, and only then tightens to NOT NULL. It creates each
replacement unique index before dropping the old one, so the tables are never
left unprotected — MariaDB commits implicitly on DDL, so ordering is the only
safety net. It runs its statements through the connection rather than addSql()
because the guard has to sit between the backfill and the NOT NULL change.

Columns are NOT NULL with no default on purpose: a construction site that
forgets assignTenant() fails at flush instead of silently writing entity_id 0,
which the phase 4 filter would then hide from everyone.

Verified on the dev database: 0 rows without a tenant, 0 personal bookings
mismatched against their doctor, 0 clinic bookings mismatched against their
clinic.

An appointment can now carry the insurance it is billed with: the service kind
(outpatient/inpatient) and the basic insurance. Confirming it no longer hands the
whole amount to the patient — the visit is split through BillingCalculator with the
coverage percent of that service kind, and the choice travels to the encounter and
the invoice built from it.

The enabled service kinds are a tenant-wide setting (all of that tenant's
insurances share it), so a tenant covering only one kind is never asked which one:
the panel resolves it the same way the server does.

- add tenant_service_category_settings + TenantServiceCategoryService, exposed on
  the existing insurance-pricing endpoint (service_categories,
  default_service_category); at least one kind must stay enabled
- add appointments.insurance_service_category / insurance_base_id with
  AppointmentInsuranceService validating them against the tenant's own settings
  and active contracts (basic only), accepted by PATCH and by confirm
- snapshot the kind on patient_sessions and invoices; the visit's coverage rule is
  resolved per kind (services keep using their own ServiceItem.service_category)
- lib/insuranceShares becomes the single client-side mirror of BillingCalculator,
  shared by the confirm modal, the appointment edit page and the session form
- surface the selection: confirm modal (with live shares), turns timeline chip,
  appointment edit page, patient record service card and invoice summary
- the session form shows the insurance block whenever the tenant has an active
  contract and prefills the patient's own insurance, so it can be changed
- fix: the confirm modal showed a zero visit price when the appointment had none —
  it now falls back to the tenant's free-visit price like the server
- fix: useServiceCategories read one level too shallow, so Persian labels never
  arrived and raw enum keys leaked into the contract summary
- fix: BlogsPage test asserted the public blogs endpoint after the page moved to
  the admin one

Three related fixes, all rooted in the same flaw: authorization and scoping
decided by the caller's role instead of by the environment the data belongs to.

1. Single-appointment access (clinic operations were entirely broken)

AppointmentController::canView/canManage only knew the patient, the owning
doctor and admin -- appointment.clinic was never consulted. A clinic user could
create an appointment through /my/appointment but got 403 on detail, edit,
move, reserve transfer/replace and status change, so nearly every appointment
operation failed in clinic mode.

AppointmentAccessChecker now decides from appointment.clinic: clinic owner,
member doctor (via ClinicDoctorPermissionChecker) and assigned secretary (via
active context + DoctorSecretary) are recognised. Actions reuse the existing
permission vocabulary, so active=false remains the single source of truth for
"collaboration ended". Cancellation is gated separately and an inline status on
PATCH /appointment/{uuid} cannot bypass that gate. The patient is narrowed to
view + cancel.

Also fixed alongside: listByDoctor now serves a clinic manager but scoped to
that clinic; todayStats gained an admin branch and no longer passes an array of
doctor ids as the clinic parameter; PatientController::appointments filters on
appointment.clinic instead of current membership, so deactivating a doctor no
longer erases clinic appointment history from the case file.

The doctor-only active_slot_key was reviewed and deliberately left alone -- a
doctor is one physical person, so adding clinic to the key would permit
double-booking, not fix a bug. Reasoning recorded on the entity.

2. Appointment registration and confirmation

Panel-created appointments are born pending ("ثبت شده") instead of confirmed.
Confirming is now an explicit act: POST /appointment/{uuid}/confirm transitions
the status, files the case file for the appointment's environment (reusing an
existing record or creating one) and registers full or partial payments on the
resulting visit -- all in one transaction.

AppointmentExpiryService would have expired those pending appointments the
moment their slot time passed; findExpiredPending is now limited to online
gateway holds, which are the only pendings carrying a TTL. A pending
appointment still occupies its slot, so the time stays reserved.

The admin panel gets a "قطعی کردن نوبت" modal showing the visit fee, each
selected service, the total, and paid/remaining/status. It is wired inside
AppointmentStatusDropdown, so picking "confirmed" anywhere (timeline, detail,
reserve list, info modal) goes through it and confirmation can never silently
skip the case file and payment.

3. Clinic case-file access

PatientRecordScopeResolver replaces the single-destination role mapping: the
active context decides, so a doctor invited into a clinic finally sees their
patients' records there. A clinic record is per-patient and shared by design,
so "their own patients" is derived from appointments with that doctor in that
clinic rather than from a new column. Clinic secretaries are limited to their
assigned doctors. Read and write share one rule, and out-of-scope records
report 404 so other environments are never disclosed.

Tests: 29 new cases across the three areas (clinic appointment access, confirm
flow, clinic record access). Full suite 466 tests, 2 pre-existing failures
unchanged. API docs updated for all three.

Two faults, one root: the per-context booking work updated ScheduleSection but
left the rest of the panel calling slot endpoints without clinic_uuid. Absent
clinic_uuid means the personal practice, so the panel asked about a schedule the
doctor barely uses and got nothing back.

- useClinicContext() resolves the current environment once and is used by the
  appointments page, useDoctorBookingServices, ServiceSlotPicker and both
  queries in NewAppointmentDrawer (a fifth call site a sweep turned up). It
  returns null in a doctor's personal environment so the mirror-image bug — a
  doctor seeing the clinic's schedule at their own practice — cannot appear.
  clinicUuid is part of every query key; without it the cache leaks across
  environments.
- appointment-slots returns empty_reason (no_schedule | holiday | day_off |
  outside_window). TurnsTimeline rendered «این روز تعطیل است» for any empty day,
  which is what the bug report actually saw; it now says which of the four it is.
- booking-locations lists a location only when the context has an address and an
  active shift points at it. The dev data had three "personal" schedules whose
  shifts referenced the clinic's address, so the public site advertised a
  personal practice that could never be booked.
- ?date= adds available_on_date per location, validated as a real calendar date.
- MyAppointmentsController and AdminApiController resolved the appointment
  address with no context and could store the wrong one. Both now go through the
  new BookingContextResolver, which also replaces AppointmentController's private
  copy of the same membership check.
- app:schedule:audit-locations reports shifts pointing at a missing or foreign
  address; --fix deactivates them rather than deleting.

Verified against the reported doctor: same date, no clinic_uuid -> 0 sessions,
with it -> 1 session; a full week matches the configured Sat/Tue/Wed/Thu.

Suite: 417 tests, 2 failures — both pre-existing and unrelated.

hamedandClaude Opus 4.8 8065ae3be1 test: enable Doctrine profiling in the test env and fix the N+1 it exposed
APP_DEBUG=0 in .env means doctrine.dbal.profiling, which defaults to
%kernel.debug%, was off in tests too, so doctrine.debug_data_holder was never
registered. Every test calling countQueries() errored out — all four N+1
regression tests had been dead for as long as they have existed. Turning
profiling on for when@test brings the harness back.

Three of the four passed immediately. The fourth was a real N+1: the
service-coverage endpoint batch-fetched its ServiceItem entities to avoid one
find() per row, but ServiceItem maps staffMembers as fetch: EAGER, so hydrating
N items fired N extra collection loads and the batch bought nothing. Six
coverage rows cost 11 queries where one row cost 6.

ServiceItemRepository::findUuidsByIds() returns the id => uuid map as a scalar
query, so no entity is hydrated and no eager collection is touched.

Also adds the query-count assertion for next_available_at that could not be
written while the harness was broken. Confirmed it fails against the previous
per-day implementation (40 queries for 2 locations, 113 for 6) and passes now.

Suite: 411 tests, 2 failures — both pre-existing and unrelated
(LowTierFixesTest, PatientWalletSessionSettleTest).

next_available_at called getAvailableSlots() once per day for up to 30 days, and
that helper re-read the schedule, holidays and overrides on every call and then
issued an isSlotTaken() query per candidate slot. Cost grew with both the days
scanned and the slots per day, multiplied by the number of locations.

findNextAvailableStart() fetches the schedule, holidays, overrides and blocking
intervals once for the whole window and walks the days in memory.

Measured on the dev data (a doctor with two locations, first opening several
days out): 73 -> 20 queries for one request. The gap widens as locations or the
distance to the first opening grow.

Reserve appointments must keep blocking here: findBusyIntervals() filters
isReserve = false, so reusing it would have reported a reserved slot as free.
Added findBlockingIntervals(), which mirrors isSlotTaken()'s predicate, and
factored both onto a shared builder.

Verified the endpoint returns identical timestamps before and after.

The API and React components were already parameterized by doctor uuid, but 14
copy-pasted identity checks limited every endpoint to "the doctor themselves or
an admin", so a clinic owner could not touch a member doctor's booking setup.

- Replaces those 14 checks with one denyDoctorAccess() that also admits the
  owner of a clinic the doctor belongs to, and a member doctor holding the
  clinic's appointment_settings permission (view for GET, update for writes).
  A doctor's own settings short-circuit before any permission lookup.
- Moves ScheduleSection and its tabs out of DoctorDetailPage into
  components/schedule/ScheduleSection.tsx so the doctor panel and the new
  clinic page render the same module instead of one page importing another.
  Pure relocation — no logic changed.
- Adds ClinicAppointmentSettingsPage: one tab per clinic doctor, each rendering
  that same section. The tab wrapper is keyed by doctor uuid so in-progress
  schedule edits cannot leak onto the wrong doctor.
- insurance-pricing accepts an optional doctor_uuid (query on GET, body on PUT)
  under the same access rule, so the visit-price card works inside the clinic
  tabs. Fixes saveInsurancePricing calling getInsurancePricing with the wrong
  argument by extracting the shared pricingPayload().

Both create endpoints (my/appointment, admin/appointment) accept
service_item_uuids[] and attach all of them. A new duration_from_services
flag gates the slot_end recompute: service-booking mode sends it true
(slot_end = start + Σ durations); slot mode omits it so the manual end
time is preserved. The admin endpoint previously ignored services entirely.

Frontend: in slot mode the single service dropdown becomes a checkbox list
filtered by the selected section (multi-select); service mode sends the
duration flag. Migration + backend/entity tests + docs updated.

Admin-side booking (POST /api/v1/my/appointment and
/api/v1/admin/appointment) resolved the patient User by mobile only, so
one person booked under two mobiles produced two User rows — and two
case-files, since PatientRecord is keyed on user_id. National code is the
real unique identity (User.national_code is already unique); a person may
have several mobiles.

Booking now requires + validates patient_national_code and resolves the
patient national-code-first (then mobile) via a shared PatientResolver, so
the case-file stays unique per national code even across mobiles. Reusing a
mobile already bound to a different national code returns 422
ERR_PROFILE_MOBILE_TAKEN. The admin create form and NewAppointmentDrawer
gain a national-code field and send it; both had a dead patient-picker URL
(/api/v1/patient) fixed to the real /api/v1/patients, whose payload already
carries user_national_code for autofill.

Docs (appointment.md, admin.md) and tests updated; new
AppointmentNationalCodeTest covers success, single-file reuse, missing,
invalid, and identity-conflict cases.

Regression: tests/Doctor/ClinicDoctorListNPlusOneTest asserts the query count
does not grow with doctor count (4→10 without the fix).

Also relaxed AppointmentExpiryServiceTest's exact-count assertion (it counts all
stale pendings in the shared db_test, which accumulates) — logged test-isolation
debt as E6.

A non-unique index on (doctor_id, slot_start) plus a count-then-insert check
left a TOCTOU race: two concurrent requests could both pass isSlotTaken and
both insert. wrapInTransaction alone doesn't stop the phantom under InnoDB
REPEATABLE-READ.

Add a nullable, unique active_slot_key on Appointment = "doctorId:slotStart"
while the booking occupies the slot (pending/confirmed — in lockstep with
isSlotTaken); NULL once expired/completed/no_show/cancelled (NULLs don't collide
in a MySQL unique index, so released slots rebook freely). bookAtomically now:
catches the unique violation -> SlotTakenException, and expires lapsed pendings
in-transaction so the ~1-min window before the expiry cron doesn't wrongly block
rebooking. All three booking paths (online / my / admin) routed through it.

Migration backfills one row per (doctor, slot) — the latest id — so the index
builds even on dirty historical data without destructively cancelling bookings.
(Backfill surfaced a real pre-existing double-booked slot in dev data.)

Regression: tests/Appointment/SlotUniquenessTest. Adjusted the expiry-service
test fixture to use distinct slots (one live booking per slot is now enforced).
