hamed	e1740462b5	perf(appointment): batch-fetch pending payments in expiry loop (fix N+1) ... AppointmentExpiryService ran one findPendingByAppointment query per expiring booking. Add PaymentRepository::findPendingByAppointments (one IN query keyed by appointment id) and use it.
