hamedandClaude Opus 5 c9d4348c46 feat(tenant): mark the financial tables with their owning environment
Phase 6 of the tenant series. GlobalTables::DEFERRED is now empty and the
coverage test asserts it stays that way.

payments carries the (entity_type, entity_id) pair and belongs to the
receiving side, never the payer: an appointment payment takes the
appointment's environment, a subscription takes the environment its buyer
owns, and an SMS wallet top-up takes the wallet's. The patient never chose
an environment, so TenantFilter stays off for them and they still see their
own payment.

Three corrections to the analysis the phase was planned on, each backed by
the code or the data rather than the plan:

- A third payment type exists. Payment::TYPE_SMS_WALLET is created in
  SmsWalletController and already carries its environment in the metadata;
  without assigning it the write would fail at flush.
- clinic_subscriptions has no user_id, and its trial rows carry no payment,
  so it cannot drive the subscription backfill. The environment is derived
  the way handleSubscriptionActivation derives it — and that method now
  reads the pair off the payment instead of re-deriving it, so a payment and
  the subscription it buys can no longer land on different environments.
- WalletTransaction is not a child of Payment. payment_id is nullable and
  none of the four creation sites set it; the wallet is a person's, with a
  running balance per user. It and Settlement, which withdraws from that same
  wallet, are global with a recorded reason instead.

bank_accounts and pos_devices move from the registering user to the
environment. Their pair is deliberately nullable: nothing in the existing
data says which of a multi-environment owner's cards belongs where, and
guessing would point real money at the wrong account. Ambiguous rows stay
unassigned and the migration reports how many. The cost is that such a row
is invisible in every environment, so the owner reaches it through a
user-scoped lookup that runs outside the filter, and assigns it with
PATCH .../{uuid}/environment. The admin panel marks those rows and offers
the assignment.

Tests: 896 backend (+11), 570 frontend (+4). PHPStan unchanged at its 17
pre-existing errors.

hamedandClaude Opus 4.8 f33c7a3eab feat(clinic-doctor): full permission coverage + enforcement, parity with secretary
The clinic-member-doctor permission system (ClinicDoctorPermission) lagged the
secretary system: only 6 resources, enforced in ~6 places, dead toggles
(services.update never checked), and a sidebar showing just appointments+patients.
Bring it to parity so a clinic owner can control exactly what each member doctor
does — while an independent doctor stays completely unrestricted.

Coverage: add insurances, addresses, inventory, tags, staff, discounts, sms to
ClinicDoctorPermission::DEFAULT_PERMISSIONS + DoctorPermissionsModal
(subscription/clinic_doctors stay owner-only by design).

New App\Clinic\Security\ClinicDoctorAccessChecker (parallel to
SecretaryAccessChecker):
- denyUnlessGranted(user, resource, action): 403 only for a clinic-member doctor
  in the clinic context; owner/admin/secretary/independent-doctor pass through.
- memberClinicId(user): resolves the member doctor to the CLINIC's tenant so the
  role-based controllers (Inventory/Tag/Staff/Discount/Sms) stop showing them
  their personal tenant in clinic context.

Enforcement wired into 10 controllers alongside the existing secretary gates:
ClinicService (services), Insurance (insurances), Patient (patients+payments),
Staff, Discount, Inventory, Tag, SmsWallet, Payment, PaymentMethod.

Frontend: the guest-doctor sidebar branch now exposes every permitted resource
(gated by can()) plus a «تنظیمات» entry; both settings navs (PurchaseSubscription
Sidebar + SETTINGS_MENU) are now permission-filtered for a scope=clinic doctor,
not just secretaries; my-payments route gets the missing payments permission.
CRUD-button gating already applies (usePermissions is role-agnostic).

Tests: ClinicDoctorPermissionEnforcementTest (member denied/allowed +
independent-doctor-unrestricted); guest-doctor sidebar gating. Backend 375 pass,
frontend 503 pass. docs/api/clinic.md updated with the full resource set +
enforcement notes.

hamedandClaude Opus 4.8 43db753942 fix(secretary): allow insurance pages; add grantable subscription resource
Insurance pages redirected to the dashboard: the insurance-pricing/claims
routes never listed `secretary`, so RoleRoute bounced a secretary who had
insurances.view and saw the menu item. Added secretary + permission
['insurances','view'] to both routes; also gated my-financial with
['payments','view'] for consistency.

«خرید اشتراک» was owner-only with no permission toggle, so it could not be
granted. Added a `subscription` secretary resource (view/create) end-to-end:
- entity DEFAULT_PERMISSIONS + SecretaryPermissions type + both secretary forms.
- backend: SubscriptionController::my (view) and trial (create),
  PaymentController::initiateSubscription (create). resolveEntity in
  SubscriptionController was already secretary-aware.
- frontend: subscription + subscription/success routes accept secretary +
  permission; settings navs gate «خرید اشتراک» by ['subscription','view'].

Tests: subscription denied-by-default / allowed-when-granted. docs/api
secretary.md updated (resource list, enforcement map, JSON example).

hamedandClaude Opus 4.8 e6422014d1 fix(appointments): file the case file on every confirmation path
Confirming an appointment was supposed to create the patient's record and its
session, and PatientService already knew how. Only two of the five paths that
confirm an appointment ever called it, and the one that mattered most did not:
a booking paid for online was confirmed inside the payment callback, which
never ran the side-effects. Every Nobat724 booking therefore went unfiled — 7
confirmed appointments in dev had no session at all.

The side-effects now run through AppointmentConfirmationService, which every
path calls: the payment callback, both PATCH endpoints, and panel/admin
bookings. Creating the record can no longer roll back a confirmation or a
payment; a failure is logged and can be repaired with the new
app:appointment:backfill-sessions command.

Two related defects fixed along the way:

- A doctor working at a clinic got two records for one appointment, one under
  the doctor and one under the clinic, so a single visit's revenue was counted
  twice. The booking context now decides, and it decides once.
- That context was inferred from address_id, falling back to "the doctor's only
  clinic" — a guess that files an appointment under the wrong practice now that
  schedules are per-context. It is stored as appointments.clinic_id instead.

Panel and admin bookings were left pending forever: nothing confirmed them and
no payment was expected. They are created confirmed.

Repeat confirmations no longer duplicate the session; an archived one still
counts as filed, so archiving a mistaken visit does not resurrect it.

hamedandClaude Opus 4.8 10be1728ff fix(payment): skip Shaparak IP allowlist for browser-redirect gateways (mellat/sep)
Mellat/SEP return the result via a user-browser redirect (POST/GET), so the
received IP is the user's, not Shaparak's. The IP allowlist therefore rejected
every real callback — including user cancel — with 'forbidden'. Security is
provided by the tamper check (RefId/SaleOrderId) and server-side verify.

- financial_breakdowns (payment_id, source) (M19) — anti double-accounting.

Regression: tests/Database/UniqueConstraintsTest (4 duplicate-insert cases).

hamedandClaude Opus 4.8 7fa4b55d3f fix(security): unique payment reference_id, reject replayed callbacks (H3)
reference_id (the gateway's settled-transaction ref) was not unique, so the
same successful callback — or a RefNum replayed onto another order — could
credit twice. Add a unique index (NULL until success, so pending/failed rows
don't collide) and an application-level pre-check in the callback that fails the
payment if the reference already belongs to another order. The unique index is
the hard backstop behind the check.

Regression: PaymentCallbackAmountTest::testReplayedGatewayReferenceIsRejected.

hamedandClaude Opus 4.8 ae06498a96 fix(security): verify gateway-confirmed amount in payment callback (C1)
The callback marked an order success on any verify-ok result without comparing
the gateway-settled amount to the amount charged. SEP returns AffectiveAmount;
an underpayment or a replayed RefNum from a cheaper order would confirm the
expensive order. Now reject (status=failed, no activation) when the gateway
reports an amount that mismatches the stored amount_rials. Gateways that don't
report a settled amount (Mellat binds it server-side) skip the check.

MockGateway now echoes mock_amount so the guard is exercisable in tests.
Regression: tests/Payment/PaymentCallbackAmountTest (underpayment rejected,
matching amount succeeds).

booking. Add PaymentRepository::findPendingByAppointments (one IN query keyed

hamedandClaude Opus 4.8 05fde81320 fix(orm): declare repositoryClass on all entities with a custom repository
On prod (opcache.preload + prod container), getRepository(Entity::class)
returned Doctrine's default repository instead of the custom one when the
entity's #[ORM\Entity] had no repositoryClass — so custom finders like
DoctorSecretaryRepository::findAllActiveBySecretary threw BadMethodCallException,
making /oauth/userinfo return 500 after login. Declare repositoryClass explicitly
on all 25 affected entities.

Also add app:create-admin command (create/promote a ROLE_ADMIN user by mobile).

hamedandClaude Opus 4.8 492a7df989 feat(payment): canceled status, manageable origin allowlist, CORS subdomains
Unify and harden the payment flow (same API for the main site and all
consumer sites; per-client difference is only frontend_address).

- Payment gains STATUS_CANCELED. Gateways distinguish user-cancel from
  failure (Mellat ResCode=17, SEP CanceledByUser, mock cancel=1) via a new
  PaymentVerifyResult::canceled flag; callback sets canceled vs failed and
  skips the circuit-breaker on cancel.
- Expiry job now cancels the pending payment when a booking lapses
  (AppointmentExpiryService + PaymentRepository::findPendingByAppointment).
- frontend_address allowlist is read from the payment_allowed_frontend_hosts
  site setting (manageable via PATCH /api/v1/admin/settings), falling back to
  the ALLOWED_FRONTEND_HOSTS env var — so a new consumer site needs no code
  change.
- .env: broaden CORS_ALLOW_ORIGIN to city subdomains (*.localhost /
  *.clinic-pro.ddev.site) and add yazd-nobat.localhost to ALLOWED_FRONTEND_HOSTS.
- Update docs/api/payment.md and docs/api/admin.md.

hamedandClaude Opus 4.8 038cb73ea7 feat(payment): add GET /api/v1/my/payments for the logged-in user
Add PaymentRepository::findByUser/countByUser and a paginated
my/payments endpoint that returns the authenticated user's own payments
(derived from the token, never a userId in the URL). Public dashboard's
transactions tab can now list payments instead of hitting a nonexistent
route.

The payment callback handled subscription and sms-wallet types but left
appointment payments pending. Add a TYPE_APPOINTMENT branch that
transitions the booking pending → confirmed (guarded by canTransitionTo,
so an already-expired booking is skipped) and dispatches a confirmation
SMS to the patient's mobile.
