hamedandClaude Opus 5 c7a3b88b32 feat(treatment): bind an appointment to a chosen session, and free it on cancel
Two holes in how a course's later appointments were made.

The link from the unbooked queue carried nothing — `/admin/appointments/new`
with no parameters — so the secretary retyped the patient and the service, and
which case the appointment joined was inferred from the service they happened to
pick. A patient with two open courses had no way to say which one they meant,
and picking the wrong service silently opened a third case. (The suggestion link
did pass slot_start and resource_uuid, but the create page never read either.)

POST /api/v1/my/appointment now takes an optional treatment_session_uuid.
SessionBookingLink validates it — same tenant, still unbooked, case open, same
patient — and reserves that session. Confirm-time attachment steps aside when
the appointment already holds a session. The booking form states in words which
session, which course and which patient it is about to book, read from a new
GET /api/v1/treatment-session/{uuid}.

Nothing ever detached a session from its appointment, so a cancelled booking
left the session `booked` forever, and since findNextUnbooked requires
"has no appointment", it could never return to the queue. Cancellation and
no-show now release it back to `planned`. A finished session is history and is
left alone.

The system still never books the next appointment by itself — it only suggests.

hamedandClaude Opus 5 9a95bc59d4 fix(appointment): make resource bookings independent of the doctor's calendar
Booking a device is not booking its doctor: the operator runs it and the doctor
only supervises. But bookAtomically locked the doctor row and isSlotTaken checked
overlap against the doctor alone, ignoring which resource was chosen, so a clinic
whose devices share one supervisor could not run two of them at once. Every
tenant in the database is in that position — clinic 2's six resources all point
at doctor 6.

Resource bookings now skip the doctor lock and carry no active_slot_key; their
guarantee comes from resource_occupancy, which understands capacity and seats.
Both direct paths write occupancy rows the way the hold engine already did, so
ResourceBookingSlotService stops being the only thing holding two sources of
truth together, and cancelling releases the seat.

Occupancy is bucketed in five-minute slices, which is coarser than a booking
time: a booking ending 12:35:04 spilled four seconds into the 12:35 bucket and
collided with the next one starting at that same second, despite zero real
overlap. This surfaced on real rows 76 and 77 during backfill. Resource bookings
now snap both ends of their window down to the bucket grid — schedule-driven
slots are already aligned, so only manually entered times move.

The seat is claimed after persist because it needs the appointment id; losing
the race removes the appointment rather than leaving a booking with no device
behind it.

app:appointment:backfill-resource-occupancy gives existing resource-backed
appointments their missing occupancy and clears the doctor keys that no longer
mean anything. It reports conflicts between two old bookings instead of picking
a loser.

hamedandClaude Opus 5 12c1d2cbf4 fix(appointment): fall back to the resource's supervising doctor on public booking
A laser device is not a doctor, so booking one from the public site sent
resource_uuid and no doctor_uuid and got back "doctor_uuid یا resource_uuid
الزامی است" — a message telling the caller to send something it had already
sent. The panel path had resolved this from ClinicResource.supervisor since it
was written; only the public path had not, and the field was defined but never
read there.

A resource with no supervisor now gets its own message pointing at the actual
fix, instead of the generic one.

hamedandClaude Opus 5 fd59eb57a3 feat(appointments): only doctors with a weekly schedule get a tab
my/clinic-doctors now reports has_schedule per doctor, and the appointments page
builds tabs from it. A doctor with no working days had a tab that could only ever
show an empty timeline.

The flag is resolved with one query for the whole list rather than one per
doctor. Clinic owners now read this authenticated endpoint too instead of the
public clinic doctor-list, which is where the flag lives; admin keeps the public
list and, with no flag present, hides nobody.

Also repairs fallout from making the resource supervisor mandatory: four test
classes build resources through their own helpers and were failing with 422. The
supervisorFor helper moved to ApiTestCase so all domains share one, rather than
copying it per suite. Full backend suite is green again (1306 tests) — the
previous commit only ran tests/Resource and missed this.

hamedandClaude Opus 5 3e3a2482fc feat(appointments): per-resource tabs backed by a resource_uuid list filter
Resources now get their own tabs on the appointments page, alongside doctors.
An appointment on "Laser CO2" belongs to the device, not to whichever doctor
happens to stand behind it, so selecting a resource tab replaces the doctor
filter instead of stacking on top of it.

GET /api/v1/my/appointments gains an optional resource_uuid filter and returns
a `resource` object per row. The join is a leftJoin on purpose: appointments
created before the resource-first model have no resource and must not drop out
of the list.

The resource tab lives in the URL so Back and refresh restore the same view,
per the list-state rule in CLAUDE.md. The doctor tab is still useState; moving
it is a separate refactor and was left untouched.

Verified against the running app: filtering by a resource returns only its
appointments, a resource from another tenant returns an empty list (TenantFilter,
200 not 403), and legacy rows still list with resource: null.

hamedandClaude Opus 5 a6acf3bfe2 Book for a resource, and manage the services a resource offers
POST /api/v1/appointment now accepts resource_uuid. When the resource is a
doctor the doctor is inferred from it, and the booking clinic is derived from
the resource's branch — sending clinic_uuid separately was only ever a way to
make the two disagree. The doctor-only path is untouched, which the public site
depends on since it sends nothing else.

Two guards before the booking is built. The resource must belong to the same
environment as the booking: it arrives as a uuid from the request body, so
TenantFilter does not cover it and without the check a patient could attach
another clinic's device to this clinic's appointment. And a resource that does
not offer the requested service is refused up front rather than discovered when
the patient turns up. That second check runs over the items the calculator
already validated rather than re-reading uuids, which is also why the
tenant-lookup inventory stays where it was.

GET and PUT /api/v1/resource/{uuid}/services manage the offerings. The list
returns the effective duration and price along with which level produced each,
so the panel can label an empty cell "30 minutes — service default" instead of
leaving the user guessing whether it is unset or zero. PUT replaces wholesale,
like the skills endpoint: a row absent from the body is a row the user removed,
and an empty string clears an override back to inheritance rather than setting
zero.

findEligible now also orders by category coverage — a device registered for
"foot" sorts ahead for a foot service. Ordering, not filtering: a clinic that
categorised only some of its devices would otherwise lose the rest.

Thirteen tests across the two files. Suite 1304 green, phpstan at its 14-error
baseline.

hamedandClaude Opus 5 03f09637ed fix(booking): store the services and duration a public booking was made with
POST /api/v1/appointment resolved the selected services, summed their minutes,
used that to compute slot_end — and then dropped the result. It never called
replaceServiceItems() or setServiceDuration(), so an appointment booked from
the public site kept no record of what it was booked for:

- the patient panel showed neither the service nor the duration
- reports counted the appointment as having no services
- a later reschedule had no duration to preserve

The management path did all of this correctly; only the public path did not.
Found by booking through the real endpoint and looking at the panel, which is
the one thing no test did.

The duration was also computed as a naive sum of duration_minutes, ignoring the
solo/additional split. That made a multi-service booking's length disagree with
the slots appointment-service-slots had just offered the patient — the booking
would occupy a different span than the one shown. Both paths now go through
ServiceBookingCalculator, which is what builds those slots.

For data that only sets duration_minutes, the calculator returns the same total
as the old sum, so existing services are unaffected.

assertServicesMatchContext() is gone: the calculator performs the identical
ownership check with the same error code and message, and the tenant-lookup
inventory is updated to match.

Tests: PublicBookingServicePersistenceTest starts at the endpoint rather than
building an appointment in memory — the gap that let this ship. Verified it
fails (4 of 8) with the fix disabled. Full suite 1433 green, slot-mode-frozen
green, phpstan at its 14-error baseline.

hamedandClaude Opus 5 4049daf071 feat: close the last four domain events, and the panel paths they describe
Every one of the fourteen named events now has an emit point. The four that
were missing all sat on paths owned by earlier tasks:

- AppointmentCompleted fires from both status-change routes, after the row is
  saved. A rejected transition or a version conflict leaves no event; otherwise
  the completed count runs ahead of the appointments themselves.
- AppointmentRescheduled is a third event, not a replacement. A rebook is a
  confirm plus a cancel, and a consumer that only hears the cancel messages a
  patient who still has an appointment.
- ResourceBlocked / ResourceReleased are a pair. Capacity coming back has to be
  as audible as capacity going away, or the resource reads as permanently taken.

Publishing is now on the scheduler rather than an unregistered command: the
logic moved out of PublishDomainEventsCommand into OutboxPublisher so the
recurring message and the manual command share it, and the existing
worker-scheduler container consumes it. The scheduler message carries no data
on purpose — what to publish is read from the table, so an event recorded
between two ticks is not skipped. DomainEventMessage routes to async, since a
slow consumer was otherwise slowing the drain itself and its failure marked a
row failed that had in fact been delivered.

Panel work that these paths made reachable:

- Cancelling from the appointment page now goes through the policy-aware
  endpoint and shows the penalty preview before the confirm, so the operator
  does not discover the patient's penalty after the fact. The cancellation
  service writes the timeline entry itself and accepts a reason, which that
  path previously dropped on the floor.
- Rescheduling reuses the booking page under ?rebook=<uuid> — the search and
  hold steps are identical and only the final step differs. The doctor picker
  is hidden there: a reschedule is not an invitation to change doctors.
- A new GET /appointment/{uuid}/segments exposes the recorded plan. An empty
  list is not an error, it means the appointment is slot-based, and that is
  exactly what gates the resource-mode reschedule button.

AppointmentInvoiceCard no longer crashes the whole detail page when an older
invoice has no discount breakdown.

Strategies (task 06 debt, task 12 dependency)
- ResourcePicker orders candidates; it deliberately does not choose. Only the
  engine knows which resource actually fits this slot and which was already
  taken by another role, and a strategy that picked would have to duplicate
  both checks
- Four implementations behind a tagged iterator: first_available (name order,
  the previous behaviour and still the default because it is predictable),
  least_gap, least_loaded, same_as_previous
- least_gap and least_loaded are deliberate opposites and both are correct;
  choosing between them is a business decision, so it lives in settings
- same_as_previous lifts a course's preferred resource to the front and keeps
  everyone else behind it. A preference, not a filter: forcing the same
  operator would make the patient wait two weeks, which is worse than a
  different operator
- Availability accepts course_uuid to supply that preference, closing the
  dependency task 12 recorded against task 06
- An unknown strategy falls back at search time but is rejected at save time.
  Stale settings must not stop bookings; a user typing a wrong value must not
  believe it took effect

Test suite flake
createUser() retries on a mobile-number collision — db_test is never reset and
holds tens of thousands of users, so the random draw does collide. The failed
INSERT closes the EntityManager, and the retry asked the container for it
again, which hands back the *same closed instance*. So the retry threw, and
every later test in that process inherited a dead manager.

That is the intermittent "EntityManager is closed" on an unrelated,
always-different test that made roughly half of full runs red and never
reproduced in a subset. Resetting the registry gives a live manager back.
UserCollisionRetryTest pins it by closing the manager on purpose.

Two consecutive full runs are green: 1334 tests.

hamedandClaude Opus 5 d56c41c87e feat(admin): resource booking mode with a readiness guard
Task 06's engine could only be switched on through the API, and nothing
checked whether the environment was ready for it. Since the mode choice is
irreversible, picking it with no resources defined would lock a clinic into a
state where no appointment is ever computable.

Backend now refuses that: resource mode requires at least one active resource,
with a message that says what to define first. Same shape as the existing
service-mode guard, applied on both save paths.

The panel shows the same conditions as a ✓/✗ list before the choice is made,
each unmet one linking to where it gets fixed — a 422 after an irreversible
decision is the wrong place to learn about a prerequisite.

Also adds the search step (minimum 5 minutes) and extends the existing mode
cards to three rather than building a parallel component.

No strategy picker: task 06 never built the strategies, and an empty menu reads
worse than an absent one.

GET /api/v1/service-items now returns has_segments, computed with one aggregate
query for the whole list rather than one per service.

hamedandClaude Opus 5 4fbedecec1 fix(booking): reserve conversion produced a zero-length midnight appointment
TransferReserveModal built the live appointment from appointment_time/end_time,
which on a reserve entry are both 00:00 because slot_start == slot_end. Moving a
reserve back to the appointment list silently created a zero-length appointment
at midnight. With the new duration validation it would now fail loudly instead.

Converting back now asks for a real time: the service picker in service mode,
two required time inputs in slot mode. The appointment -> reserve direction is
untouched.

GET /my/appointments has its own array-hydration serializer rather than
Appointment::toArray(), so it exposed none of the service fields the panel needs.
Added service_items (separate query, no row multiplication and no N+1),
clinic_uuid and the duration pair. This was also a hidden prerequisite of the
public-site task, whose checklist listed it as "verify first".

The reserve table now lists every service instead of only the first.

Not done, deliberately: the DataTable migration the task asked for. Its stated
reason — inline tokens breaking dark mode — does not hold; this table's th/td
already use CSS variables and dark mode works. Rewriting a working table for no
real gain is unjustified risk.

hamedandClaude Opus 5 50759bf663 feat(admin): service-aware time picking on the appointment edit page
In service mode the page now mounts the existing ServiceSlotPicker and hides the
three free-form time inputs plus the single-service select: a 45-minute service
could previously be shortened to 20 and the next patient would sit on top of it.
Hidden rather than disabled — a disabled field reads as "you must do something
here".

Saving splits in two: the service-aware endpoint takes the time and services
(the client sends no duration), then the usual PATCH carries deposit, insurance,
status and note without slot_start/slot_end/version, since the reschedule already
advanced the optimistic-lock version.

Booking mode is read from the appointment's own schedule via an explicit
clinic_uuid, not from the panel's current environment: a doctor can be slot-based
in their office and service-based in a clinic. That required exposing clinic_uuid
in Appointment::toArray(), which was missing.

appointment-service-slots accepts exclude_appointment_uuid, gated on canManage of
that appointment — an ungated parameter would let anyone fabricate availability.

ServiceSlotPicker gained two optional props; its existing callers pass neither and
are unaffected. Its reset-on-doctor-change effect now skips the first run, which
would otherwise wipe the initial selection.

POST /api/v1/appointment/{uuid}/service-reschedule takes only a start time and
derives the length from the appointment's services. PATCH also validates the
duration, but the client must already know the correct slot_end; not needing that
knowledge is what lets the edit form drop its manual time inputs.

The start must be a member of getServiceStartTimes(), not merely free:
isSlotTaken() reports collisions with other appointments, while the offered list
also applies shift bounds, holidays, date overrides, the booking window and the
buffer. Without it a secretary could park an appointment at 3am.

forManagement comes from canManageContext(), not canManage(): a patient moving
their own appointment must still respect the public booking window.

hamedandClaude Opus 5 b589a851d0 feat(booking): make PATCH derive appointment duration from its services
In service mode PATCH accepted any duration and only updated the single
service_item column while the service_items collection stayed untouched, so an
edit could leave an appointment with old services and a new length. A 45-minute
service could be shortened to 20 and the next patient would sit on top of it.

Services are now resolved before the time block (duration depends on them) and
the stored end must equal start + total minutes. Reserve entries are exempt:
they carry slot_start == slot_end and occupy no interval, but they do store the
computed duration so a later conversion does not lose it.

No convert-reserve endpoint was added: PATCH already converts a reserve to a
timed appointment via rescheduleTo($start, $end, $isReserve), which refreshes
active_slot_key itself. Project rule 8 — a new endpoint needs an existing one to
be insufficient even after extension.

Slot mode is untouched: with booking_mode = slot the duration stays null and not
one of the new branches runs. Covered by an explicit test.

New error codes are ERR_APPOINTMENT_003/004 (the file only had 001/002).

hamedandClaude Opus 5 6afc5c090e refactor(booking): extract ServiceBookingCalculator from the controller
"Allowed duration of a service combination" lived inside
AppointmentController::serviceSlots(). Three upcoming callers need the same
computation (PATCH duration validation, service-aware reschedule, reserve
conversion); copying it would mean four variants with four different edge-case
behaviours.

The extraction is behaviour-preserving: BaseController::error() and
ExceptionSubscriber emit an identical envelope, so returning $this->error() was
replaced by throwing AppException with the same code/message/field.

Tenant ownership now goes through TenantOwnershipChecker::belongsToPair() (the
documented single point) instead of an inline section pair comparison. The repo
property is named itemRepo on purpose: TenantLookupInventoryTest only counts
recognised property names, so any other name would slip past the safety net.

The naive duration sum is kept deliberately — switching to solo/additional
minutes is task 04 and changes one line here.

hamedandClaude Opus 5 d2f4b5c428 fix(tenant): check the environment wherever a uuid comes from the request
Phase 7 was scoped to guard aggregate children, which the Doctrine filter cannot
reach. Measuring first — as the plan required — moved the target: all 22 children
and their 20 repositories were already sound. Every list query anchors on its
root, and ServiceItemRepository even joins service_sections and filters on the
pair by hand. A repository-level guard would have found nothing.

The real exposure was one layer up. Where a uuid arrives from a request body or
query string, the entity it names is loaded by uuid alone, and the filter is no
help: aggregate children have no tenant column, and a panel user who never chose
an environment is not filtered at all. Three leaks, each proven by removing the
fix and watching the new tests go red:

- GET /api/v1/appointment-service-slots accepted service_item_uuids from any
  environment. Existence, bookable state and duration leaked through the error
  messages and the returned slots. The booking path in the same controller had
  guarded this since it was written; the slot path never did.
- POST /api/v1/my/appointment attached service_section_uuid, service_item_uuid,
  staff_uuid and the service list without any check, and persisted them onto the
  appointment. A write, not just a read.
- PatientService did the same in all three of its loops — pricing, session
  create, session update — so another environment's service price entered the
  invoice and its SessionService row was stored, staff included.

TenantOwnershipChecker is the single place that answers "does this belong to the
current environment?". It reads getEntityType()/getEntityId(), so ServiceItem now
delegates that pair to its section: an aggregate child exposing the tenant it
inherits. An entity that exposes no pair throws rather than returning false —
silence here builds an always-closed guard, which is its own bug.

TenantLookupInventoryTest keeps a per-file count of these lookups. It earned its
place immediately: the first run found more sites than the manual grep had, and
reviewing them turned up the third PatientService loop. StaffController looked
unguarded until read properly — ownsStaff sits two lines below the null check.

One assertion was wrong before it was right: the create-path test read
`$session['services'] ?? []`, which passes vacuously. It now counts the stored
rows through the repository, and fails without the fix.

Tests: 879 passing. PHPStan unchanged at its 17 pre-existing errors.

Phase 2 of the tenant-marking series. appointments, weekly_schedules and
date_overrides kept their environment implicit in a nullable clinic_id, so
every query that wanted "this environment's rows" had to rebuild
clinic_id IS NULL ? doctor : clinic itself. The two calendar tables also
depended on a MariaDB-only generated column, clinic_key = IFNULL(clinic_id, 0),
purely to make a unique key work across NULLs.

All three now carry the (entity_type, entity_id) pair that service_sections,
patient_records and clinic_staff already use, via a shared TenantOwnedTrait.
The pair is a deliberate denormalisation of clinic_id/doctor_id: the automatic
tenant filter and the tenant-leading indexes both need a real column, and
neither can be built on an IF() expression.

- Unique keys keep doctor_id alongside the pair. A clinic has several doctors
  and each has their own schedule, so (entity_type, entity_id) alone would
  reject the second doctor.
- clinic_key is gone from both calendar tables.
- appointments gained tenant-leading indexes; EXPLAIN on the panel's list query
  now picks idx_appointments_tenant_slot.

Deliberately unchanged, both with the reason already recorded in the code:
active_slot_key stays keyed on doctor + slot, since adding the environment
would let one doctor be booked in their own practice and a clinic at the same
moment. holidays keeps its nullable clinic_id, where NULL means "every
environment" rather than "personal practice" — a meaning the pair cannot carry.

The migration adds the columns nullable, backfills, aborts if any row is left
without an owner, and only then tightens to NOT NULL. It creates each
replacement unique index before dropping the old one, so the tables are never
left unprotected — MariaDB commits implicitly on DDL, so ordering is the only
safety net. It runs its statements through the connection rather than addSql()
because the guard has to sit between the backfill and the NOT NULL change.

Columns are NOT NULL with no default on purpose: a construction site that
forgets assignTenant() fails at flush instead of silently writing entity_id 0,
which the phase 4 filter would then hide from everyone.

Verified on the dev database: 0 rows without a tenant, 0 personal bookings
mismatched against their doctor, 0 clinic bookings mismatched against their
clinic.

Tests: 819 passing (813 + 6 new in BookingTenantTest). PHPStan clean on every
changed file.

hamedandClaude Opus 5 1a7bf53577 refactor(tenant): make EntityContextResolver the single context resolver
Phase 1 of the tenant-marking series. The "which environment is this user
working in?" decision was reimplemented in six places, each reading
UserActiveContext.db_uuid and then guessing whether the uuid belongs to a
clinic or a doctor. Every copy was a place the roles could silently diverge.

EntityContextResolver already encoded the right precedence (explicit
clinic_uuid > stored active context > role) but only five files used it, and
it did not recognise secretaries at all: canActInClinic accepted admins,
clinic owners and member doctors, so a secretary's active clinic context
always collapsed to unknown. That gap is why SecretaryAccessChecker carried
its own copy of the logic.

- canActInClinic now also accepts an active DoctorSecretary relation, and a
  matching canActForDoctor covers the personal-practice branch.
- AppointmentAccessChecker, ClinicDoctorAccessChecker, SecretaryAccessChecker,
  PatientRecordScopeResolver, MyAppointmentsController and the secretary
  dashboard all resolve through it now.
- PatientRecordScopeResolver keeps only its real responsibility: which
  doctors' patients are visible inside the resolved environment.
- The resolver answers "where"; ClinicDoctorPermissionChecker and
  SecretaryPermissionChecker still answer "what may you do".

Left deliberately untouched, with the reason recorded at each site:
SubscriptionController, InventoryController and TenantTagController check
ROLE_DOCTOR unconditionally and ignore the active context, so a member doctor
sees personal inventory/tags/subscription even inside a clinic. Switching them
changes what users see, which is a product decision, not a refactor.
AuthController keeps its repository because it writes the active context.

tests/ApiTestCase now seeds the "free" subscription plan. db_test had no such
row, so getEffectivePlan returned null, every hasFeature() was false and 83
tests across Patient, ClinicService, Insurance and Appointment failed with 403.

No schema, route, request, response or error code changed.

Tests: 813 passing (was 730 passing / 83 failing). PHPStan clean on all
changed files.

An appointment can now carry the insurance it is billed with: the service kind
(outpatient/inpatient) and the basic insurance. Confirming it no longer hands the
whole amount to the patient — the visit is split through BillingCalculator with the
coverage percent of that service kind, and the choice travels to the encounter and
the invoice built from it.

The enabled service kinds are a tenant-wide setting (all of that tenant's
insurances share it), so a tenant covering only one kind is never asked which one:
the panel resolves it the same way the server does.

- add tenant_service_category_settings + TenantServiceCategoryService, exposed on
  the existing insurance-pricing endpoint (service_categories,
  default_service_category); at least one kind must stay enabled
- add appointments.insurance_service_category / insurance_base_id with
  AppointmentInsuranceService validating them against the tenant's own settings
  and active contracts (basic only), accepted by PATCH and by confirm
- snapshot the kind on patient_sessions and invoices; the visit's coverage rule is
  resolved per kind (services keep using their own ServiceItem.service_category)
- lib/insuranceShares becomes the single client-side mirror of BillingCalculator,
  shared by the confirm modal, the appointment edit page and the session form
- surface the selection: confirm modal (with live shares), turns timeline chip,
  appointment edit page, patient record service card and invoice summary
- the session form shows the insurance block whenever the tenant has an active
  contract and prefills the patient's own insurance, so it can be changed

1) Settings menu structure. Phase B flat-listed staff/discounts/sms/tags/

   the doctor/clinic layout instead: only inventory + services stay in the main
   «مدیریت» nav; the rest live under a single «تنظیمات» entry
   (→ /admin/account-settings). Made both settings navs permission-aware for
   secretaries: SETTINGS_MENU (menuForRole now takes `can`) and
   PurchaseSubscriptionSidebar filter by a per-item `perm`/`alwaysOpen` instead
   of role only, so a secretary sees exactly their permitted settings pages and
   owner-only items (subscription, secretary-management) stay hidden.

2) Clinic secretary appointment timeline. AppointmentsPage treated a
   clinic-scoped secretary as a single-doctor profile: the doctor list was
   fetched/shown only for isClinic/isAdmin, so no doctor tabs, timeline, or
   booking. Now a clinic-scoped secretary is multi-doctor: fetches the doctor
   list, shows tabs, auto-selects the first doctor. The list comes from a new
   authenticated endpoint GET /api/v1/my/clinic-doctors returning only the
   secretary's ASSIGNED doctors — /clinic/doctor-list is on the public (no-JWT)
   firewall and cannot scope by user, so it would have leaked unbookable doctors.

3) Patient record delete. The `patients.delete` toggle was dead: every record
   delete (note/medical-record/attachment/call/message) was gated as
   `patients.update`. Mapped them to `patients.delete` so the toggle is honored
   and delete is controllable separately from edit.

New SecretaryAccessChecker::assignedClinicDoctorIds. Tests: doctor-list scoping,
patients.delete separation (denied/allowed). docs/api secretary.md +
appointment.md updated. Backend 286 + frontend 25 pass.

Backend enforcement (SecretaryAccessChecker, three new reusable helpers):
- resolveOwnerEntity(): owner pair from active context — used by StaffController,
  DiscountController, SmsWalletController (now secretary-aware resolveEntity).
- canForDoctor(): per-doctor-scoped check (assigned doctor + toggle) — wired into
  AppointmentSettingsController::denyDoctorAccess.
- canForClinic(): clinic-scoped check — wired into ClinicController::detachDoctor,
  ClinicDoctorPermissionController (view/update), ClinicInvitationController
  (create/view/update/delete). clinic_doctors is clinic-context only.
Guards run ahead of any subscription gate; non-secretary roles pass unchanged.

Frontend:
- RoleRoute: staff, discounts, sms-wallet, appointment-settings (doctor+clinic
  variants), settings/clinic-doctors routes accept secretary + permission gate.
- Sidebar (secretary branch): five new items gated by can(); appointment_settings
  route follows active scope; clinic_doctors only in clinic scope.

Tests: SecretaryResourceEnforcementTest — denied-by-default + allowed-when-granted
for all five (18 total). Sidebar.test — B-resource gating + clinic_doctors scope
rule. docs/api/secretary.md resource list, enforcement map, JSON example updated.

Three related fixes, all rooted in the same flaw: authorization and scoping
decided by the caller's role instead of by the environment the data belongs to.

1. Single-appointment access (clinic operations were entirely broken)

AppointmentController::canView/canManage only knew the patient, the owning
doctor and admin -- appointment.clinic was never consulted. A clinic user could
create an appointment through /my/appointment but got 403 on detail, edit,
move, reserve transfer/replace and status change, so nearly every appointment
operation failed in clinic mode.

AppointmentAccessChecker now decides from appointment.clinic: clinic owner,
member doctor (via ClinicDoctorPermissionChecker) and assigned secretary (via
active context + DoctorSecretary) are recognised. Actions reuse the existing
permission vocabulary, so active=false remains the single source of truth for
"collaboration ended". Cancellation is gated separately and an inline status on
PATCH /appointment/{uuid} cannot bypass that gate. The patient is narrowed to
view + cancel.

Also fixed alongside: listByDoctor now serves a clinic manager but scoped to
that clinic; todayStats gained an admin branch and no longer passes an array of
doctor ids as the clinic parameter; PatientController::appointments filters on
appointment.clinic instead of current membership, so deactivating a doctor no
longer erases clinic appointment history from the case file.

The doctor-only active_slot_key was reviewed and deliberately left alone -- a
doctor is one physical person, so adding clinic to the key would permit
double-booking, not fix a bug. Reasoning recorded on the entity.

2. Appointment registration and confirmation

Panel-created appointments are born pending ("ثبت شده") instead of confirmed.
Confirming is now an explicit act: POST /appointment/{uuid}/confirm transitions
the status, files the case file for the appointment's environment (reusing an
existing record or creating one) and registers full or partial payments on the
resulting visit -- all in one transaction.

AppointmentExpiryService would have expired those pending appointments the
moment their slot time passed; findExpiredPending is now limited to online
gateway holds, which are the only pendings carrying a TTL. A pending
appointment still occupies its slot, so the time stays reserved.

The admin panel gets a "قطعی کردن نوبت" modal showing the visit fee, each
selected service, the total, and paid/remaining/status. It is wired inside
AppointmentStatusDropdown, so picking "confirmed" anywhere (timeline, detail,
reserve list, info modal) goes through it and confirmation can never silently
skip the case file and payment.

3. Clinic case-file access

PatientRecordScopeResolver replaces the single-destination role mapping: the
active context decides, so a doctor invited into a clinic finally sees their
patients' records there. A clinic record is per-patient and shared by design,
so "their own patients" is derived from appointments with that doctor in that
clinic rather than from a new column. Clinic secretaries are limited to their
assigned doctors. Read and write share one rule, and out-of-scope records
report 404 so other environments are never disclosed.

Tests: 29 new cases across the three areas (clinic appointment access, confirm
flow, clinic record access). Full suite 466 tests, 2 pre-existing failures
unchanged. API docs updated for all three.

hamedandClaude Opus 4.8 e6422014d1 fix(appointments): file the case file on every confirmation path
Confirming an appointment was supposed to create the patient's record and its
session, and PatientService already knew how. Only two of the five paths that
confirm an appointment ever called it, and the one that mattered most did not:
a booking paid for online was confirmed inside the payment callback, which
never ran the side-effects. Every Nobat724 booking therefore went unfiled — 7
confirmed appointments in dev had no session at all.

The side-effects now run through AppointmentConfirmationService, which every
path calls: the payment callback, both PATCH endpoints, and panel/admin
bookings. Creating the record can no longer roll back a confirmation or a
payment; a failure is logged and can be repaired with the new
app:appointment:backfill-sessions command.

Two related defects fixed along the way:

- A doctor working at a clinic got two records for one appointment, one under
  the doctor and one under the clinic, so a single visit's revenue was counted
  twice. The booking context now decides, and it decides once.
- That context was inferred from address_id, falling back to "the doctor's only
  clinic" — a guess that files an appointment under the wrong practice now that
  schedules are per-context. It is stored as appointments.clinic_id instead.

Panel and admin bookings were left pending forever: nothing confirmed them and
no payment was expected. They are created confirmed.

Repeat confirmations no longer duplicate the session; an archived one still
counts as filed, so archiving a mistaken visit does not resurrect it.

Two faults, one root: the per-context booking work updated ScheduleSection but
left the rest of the panel calling slot endpoints without clinic_uuid. Absent
clinic_uuid means the personal practice, so the panel asked about a schedule the
doctor barely uses and got nothing back.

- useClinicContext() resolves the current environment once and is used by the
  appointments page, useDoctorBookingServices, ServiceSlotPicker and both
  queries in NewAppointmentDrawer (a fifth call site a sweep turned up). It
  returns null in a doctor's personal environment so the mirror-image bug — a
  doctor seeing the clinic's schedule at their own practice — cannot appear.
  clinicUuid is part of every query key; without it the cache leaks across
  environments.
- appointment-slots returns empty_reason (no_schedule | holiday | day_off |
  outside_window). TurnsTimeline rendered «این روز تعطیل است» for any empty day,
  which is what the bug report actually saw; it now says which of the four it is.
- booking-locations lists a location only when the context has an address and an
  active shift points at it. The dev data had three "personal" schedules whose
  shifts referenced the clinic's address, so the public site advertised a
  personal practice that could never be booked.
- ?date= adds available_on_date per location, validated as a real calendar date.
- MyAppointmentsController and AdminApiController resolved the appointment
  address with no context and could store the wrong one. Both now go through the
  new BookingContextResolver, which also replaces AppointmentController's private
  copy of the same membership check.
- app:schedule:audit-locations reports shifts pointing at a missing or foreign
  address; --fix deactivates them rather than deleting.

Verified against the reported doctor: same date, no clinic_uuid -> 0 sessions,
with it -> 1 session; a full week matches the configured Sat/Tue/Wed/Thu.

Suite: 417 tests, 2 failures — both pre-existing and unrelated.

next_available_at called getAvailableSlots() once per day for up to 30 days, and
that helper re-read the schedule, holidays and overrides on every call and then
issued an isSlotTaken() query per candidate slot. Cost grew with both the days
scanned and the slots per day, multiplied by the number of locations.

findNextAvailableStart() fetches the schedule, holidays, overrides and blocking
intervals once for the whole window and walks the days in memory.

Measured on the dev data (a doctor with two locations, first opening several
days out): 73 -> 20 queries for one request. The gap widens as locations or the
distance to the first opening grow.

Reserve appointments must keep blocking here: findBusyIntervals() filters
isReserve = false, so reusing it would have reported a reserved slot as free.
Added findBlockingIntervals(), which mirrors isSlotTaken()'s predicate, and
factored both onto a shared builder.

Verified the endpoint returns identical timestamps before and after.

  that same section. The tab wrapper is keyed by doctor uuid so in-progress

  under the same access rule, so the visit-price card works inside the clinic
  tabs. Fixes saveInsurancePricing calling getInsurancePricing with the wrong

changing the service default: appointment-service-slots accepts durations[uuid]
and both create endpoints accept service_durations; the override drives total
duration and slot_end. Online (patient) booking is unaffected — it never sends
overrides. Backend + frontend tests and docs updated.

Appointments could only reference a single service (ManyToOne). Add an
appointment_service_items join table (ManyToMany) so an appointment can
carry several services; the first stays the primary service_item for
backward compatibility, and toArray now also returns service_items[].

Both create endpoints (my/appointment, admin/appointment) accept
service_item_uuids[] and attach all of them. A new duration_from_services
flag gates the slot_end recompute: service-booking mode sends it true
(slot_end = start + Σ durations); slot mode omits it so the manual end
time is preserved. The admin endpoint previously ignored services entirely.

Frontend: in slot mode the single service dropdown becomes a checkbox list
filtered by the selected section (multi-select); service mode sends the
duration flag. Migration + backend/entity tests + docs updated.

- Introduced a new booking mode in WeeklySchedule to support service-based appointments.
- Updated SlotCalculatorService to calculate available start times based on selected service durations and buffer times.

- Implemented validation to ensure at least one bookable service exists for doctors in service mode.
