Strategies (task 06 debt, task 12 dependency)
- ResourcePicker orders candidates; it deliberately does not choose. Only the
engine knows which resource actually fits this slot and which was already
taken by another role, and a strategy that picked would have to duplicate
both checks
- Four implementations behind a tagged iterator: first_available (name order,
the previous behaviour and still the default because it is predictable),
least_gap, least_loaded, same_as_previous
- least_gap and least_loaded are deliberate opposites and both are correct;
choosing between them is a business decision, so it lives in settings
- same_as_previous lifts a course's preferred resource to the front and keeps
everyone else behind it. A preference, not a filter: forcing the same
operator would make the patient wait two weeks, which is worse than a
different operator
- Availability accepts course_uuid to supply that preference, closing the
dependency task 12 recorded against task 06
- An unknown strategy falls back at search time but is rejected at save time.
Stale settings must not stop bookings; a user typing a wrong value must not
believe it took effect
Test suite flake
createUser() retries on a mobile-number collision — db_test is never reset and
holds tens of thousands of users, so the random draw does collide. The failed
INSERT closes the EntityManager, and the retry asked the container for it
again, which hands back the *same closed instance*. So the retry threw, and
every later test in that process inherited a dead manager.
That is the intermittent "EntityManager is closed" on an unrelated,
always-different test that made roughly half of full runs red and never
reproduced in a subset. Resetting the registry gives a live manager back.
UserCollisionRetryTest pins it by closing the manager on purpose.
Two consecutive full runs are green: 1334 tests.

Task 06's engine could only be switched on through the API, and nothing
checked whether the environment was ready for it. Since the mode choice is
irreversible, picking it with no resources defined would lock a clinic into a
state where no appointment is ever computable.
Backend now refuses that: resource mode requires at least one active resource,
with a message that says what to define first. Same shape as the existing
service-mode guard, applied on both save paths.
The panel shows the same conditions as a ✓/✗ list before the choice is made,
each unmet one linking to where it gets fixed — a 422 after an irreversible
decision is the wrong place to learn about a prerequisite.
Also adds the search step (minimum 5 minutes) and extends the existing mode
cards to three rather than building a parallel component.
No strategy picker: task 06 never built the strategies, and an empty menu reads
worse than an absent one.
GET /api/v1/service-items now returns has_segments, computed with one aggregate
query for the whole list rather than one per service.

Phase 2 of the tenant-marking series. appointments, weekly_schedules and
date_overrides kept their environment implicit in a nullable clinic_id, so
every query that wanted "this environment's rows" had to rebuild
clinic_id IS NULL ? doctor : clinic itself. The two calendar tables also
depended on a MariaDB-only generated column, clinic_key = IFNULL(clinic_id, 0),
purely to make a unique key work across NULLs.
All three now carry the (entity_type, entity_id) pair that service_sections,
patient_records and clinic_staff already use, via a shared TenantOwnedTrait.
The pair is a deliberate denormalisation of clinic_id/doctor_id: the automatic
tenant filter and the tenant-leading indexes both need a real column, and
neither can be built on an IF() expression.
- Unique keys keep doctor_id alongside the pair. A clinic has several doctors
and each has their own schedule, so (entity_type, entity_id) alone would
reject the second doctor.
- clinic_key is gone from both calendar tables.
- appointments gained tenant-leading indexes; EXPLAIN on the panel's list query
now picks idx_appointments_tenant_slot.
Deliberately unchanged, both with the reason already recorded in the code:
active_slot_key stays keyed on doctor + slot, since adding the environment
would let one doctor be booked in their own practice and a clinic at the same
moment. holidays keeps its nullable clinic_id, where NULL means "every
environment" rather than "personal practice" — a meaning the pair cannot carry.
The migration adds the columns nullable, backfills, aborts if any row is left
without an owner, and only then tightens to NOT NULL. It creates each
replacement unique index before dropping the old one, so the tables are never
left unprotected — MariaDB commits implicitly on DDL, so ordering is the only
safety net. It runs its statements through the connection rather than addSql()
because the guard has to sit between the backfill and the NOT NULL change.
Columns are NOT NULL with no default on purpose: a construction site that
forgets assignTenant() fails at flush instead of silently writing entity_id 0,
which the phase 4 filter would then hide from everyone.
Verified on the dev database: 0 rows without a tenant, 0 personal bookings
mismatched against their doctor, 0 clinic bookings mismatched against their
clinic.
Tests: 819 passing (813 + 6 new in BookingTenantTest). PHPStan clean on every
changed file.

Extends the secretary permission system to five previously owner-only modules,
so a clinic/doctor can delegate each page to a secretary. All were unreachable
by secretaries before (role-based tenant resolution returned "unknown" → 403).
New permission resources (default-deny, three-place add: entity default,
SecretaryPermissions type, both MySecretariesPage + admin SecretariesPage):
staff, discounts, sms, appointment_settings (view/update only), clinic_doctors
(clinic-only — hidden from independent doctors via `clinicOnly` section filter).
Backend enforcement (SecretaryAccessChecker, three new reusable helpers):
- resolveOwnerEntity(): owner pair from active context — used by StaffController,
DiscountController, SmsWalletController (now secretary-aware resolveEntity).
- canForDoctor(): per-doctor-scoped check (assigned doctor + toggle) — wired into
AppointmentSettingsController::denyDoctorAccess.
- canForClinic(): clinic-scoped check — wired into ClinicController::detachDoctor,
ClinicDoctorPermissionController (view/update), ClinicInvitationController
(create/view/update/delete). clinic_doctors is clinic-context only.
Guards run ahead of any subscription gate; non-secretary roles pass unchanged.
Frontend:
- RoleRoute: staff, discounts, sms-wallet, appointment-settings (doctor+clinic
variants), settings/clinic-doctors routes accept secretary + permission gate.
- Sidebar (secretary branch): five new items gated by can(); appointment_settings
route follows active scope; clinic_doctors only in clinic scope.
Tests: SecretaryResourceEnforcementTest — denied-by-default + allowed-when-granted
for all five (18 total). Sidebar.test — B-resource gating + clinic_doctors scope
rule. docs/api/secretary.md resource list, enforcement map, JSON example updated.

The API and React components were already parameterized by doctor uuid, but 14
copy-pasted identity checks limited every endpoint to "the doctor themselves or
an admin", so a clinic owner could not touch a member doctor's booking setup.
- Replaces those 14 checks with one denyDoctorAccess() that also admits the
owner of a clinic the doctor belongs to, and a member doctor holding the
clinic's appointment_settings permission (view for GET, update for writes).
A doctor's own settings short-circuit before any permission lookup.
- Moves ScheduleSection and its tabs out of DoctorDetailPage into
components/schedule/ScheduleSection.tsx so the doctor panel and the new
clinic page render the same module instead of one page importing another.
Pure relocation — no logic changed.
- Adds ClinicAppointmentSettingsPage: one tab per clinic doctor, each rendering
that same section. The tab wrapper is keyed by doctor uuid so in-progress
schedule edits cannot leak onto the wrong doctor.
- insurance-pricing accepts an optional doctor_uuid (query on GET, body on PUT)
under the same access rule, so the visit-price card works inside the clinic
tabs. Fixes saveInsurancePricing calling getInsurancePricing with the wrong
argument by extracting the shared pricingPayload().

getOverride leaked any doctor's override to any authenticated user; add the
owner-or-admin check (matching the update/delete endpoints) + regression test.
getSchedule returned any doctor's schedule to any authenticated user — the
mutation endpoints (update/delete) already checked owner-or-admin but this GET
did not. Add the same check + regression test (fails without the fix).
Appointments now persist address_id resolved from the weekly-schedule
session (location_id) across all booking paths (online, secretary, admin).
On confirm, the patient is added to the clinic owning that address, or to
the doctor's single clinic as fallback. Weekly-schedule create/update now
requires location_id on every active session. PatientSession exposes
doctor_uuid/doctor_name so clinic records show which doctor each visit is for.

Add a reserved meta key inside WeeklySchedule.setting (no migration) holding
online_booking_enabled and a booking_window value+unit (week|month), with
sane defaults. Expose meta separately in toArray() and keep it out of the
day schedule map. setSetting now preserves meta when the day schedule is
replaced; the weekly-schedule POST/PATCH endpoints accept an optional meta.
