A representative could create a doctor or clinic but not finish its profile:
PATCH /api/v1/doctor/{uuid} accepted only the doctor or an admin, and the
clinic gate ran through ClinicDoctorPermissionChecker, which asks about clinic
membership — a representative is not a member. Onboarding stopped at an empty
public record.
Grant is permanent while representation_id points at the rep, and limited to
content: RepresentationEditPolicy holds ownership plus the field whitelist.
Sending a key outside it aborts the whole request with 403 and names the field,
rather than filtering the payload silently, so a rep never believes a change
saved when it did not. medical_system_code, `active` and clinic `doctors` stay
out — credential, and membership, belong to the record's owner. `active` already
has a dedicated rep endpoint.
ClinicDoctorPermissionChecker is untouched on purpose; folding a second concept
into it would give it two reasons to change.
Doctor/clinic detail responses now carry can_edit, computed by the same policy
the PATCH gate uses, so the panel reads authorization instead of re-deriving it
and drifting. Both endpoints stay public: no token means can_edit false and an
otherwise unchanged payload, which is what nobat724_front consumes.
Address endpoints follow the same policy. createAddress now resolves its target
from an explicit doctor_uuid instead of findByUser first — a representative who
also has a doctor profile was silently writing the address onto their own.
Every rep edit writes one app_log row (channel representation_edit) recording
who, what, and which field names — never values. Owner and admin edits write
nothing, keeping /admin/logs readable.
Docs corrected where they already disagreed with the code: 403/404 error codes
on both PATCH routes, a non-existent "cannot delete the last clinic address"
409, and the missing gallery-size 422.

PatientController::resolveScope and TenantTagController::guardTagView only ever
checked the secretary, while every write in both controllers already ran through
both checkers. So an invited clinic doctor with patients.view off got 200 with an
empty list where a secretary got 403 — one permission, two behaviours. No data
was exposed either way; tenant scoping emptied the result.
The fix is not canOrNonMember. That collapses two different situations: a
membership row switched to active=false means the collaboration ended, and
ClinicDoctorPermission::can() returns false for everything in that case too.
Routing it through the permission gate turned the existing 404 on a single record
into a 403, which confirms the record exists to someone who just lost access.
ClinicRecordAccessTest caught it.
isActiveMemberDenied() answers the narrower question — active member, permission
off — and leaves a deactivated row to the data scope, which closes it with a 404
and discloses nothing. A test now pins that distinction so it cannot be collapsed
again.
Tags keep the tags.view OR patients.view rule, now for both roles.
Verified live in three states: active with both off 403/403, deactivated not 403,
active with patients.view on 200/200.

A full role-by-role sweep (9 roles x 18 endpoints against the running app) showed
the addresses toggles in the owner's permission form controlled nothing. Grep
confirms it: no gate anywhere referenced 'addresses'. The panel's address list was
gated on appointment_settings.view instead — the same borrowed-permission pattern
already fixed for resources and treatment.
GET /api/v1/addresses now gates on addresses.view.
The resource drops to view-only. Creating, updating and deleting an address in
ClinicController is explicitly owner-or-admin
($clinic->getUser()->getId() !== $user->getId()), so those three actions could
never be delegated to a secretary or an invited doctor no matter what the form
said. Both role defaults narrow to ['view' => true] to match, and stored JSON
keeps its old keys harmlessly since merge only reads registry keys.
This widens secretary access: addresses.view defaults to true while
appointment_settings.view defaults to false, so secretaries who could not list
addresses now can. That is deliberate and costs no confidentiality — the same
addresses are already served anonymously from
GET /api/v1/clinic/{uuid}/addresses, which is whitelisted in security.yaml.
Verified live in three states: default 200, addresses.view off 403, and
addresses off with appointment_settings on still 403, proving the borrow is gone.

The list of permissionable resources was duplicated in six places that had
already diverged: both permission entities, three admin UI files and the
SecretaryPermissions TypeScript interface. Adding a resource meant editing all
of them, so new pages borrowed an unrelated resource instead — five resource
pages sat on appointment_settings.view and treatment-cases on appointments.view.
PermissionCatalog is now the only place that says which resources and actions
exist. Each entity keeps its own DEFAULT_PERMISSIONS, but as role policy only;
a test asserts those defaults never name a resource the registry doesn't have.
getPermissions() merges the stored JSON over the role defaults, so a resource
added to the registry later resolves to the role default instead of silently
false for every existing row. Explicitly stored values are never overwritten,
and no data migration is needed.
Two asymmetries fixed along the way:
- ClinicDoctorPermission validated writes against its own DEFAULT_PERMISSIONS,
so services.create/delete could never be stored for an invited doctor.
- DoctorSecretary had no validation at all and would store any key, and it only
read $patch['resources'] — the admin SecretariesPage sends a flat map, so its
permission edit silently did nothing. Both entities now accept either shape
and filter through the registry.
New resources 'resources' and 'treatment' are registered with defaults chosen to
preserve today's effective access, since both pages are currently gated on a
borrowed resource.

A practice domain is the field a clinic operates in — beauty, dentistry —
and unlike Specialty it is configuration, not a label: treatment workflows
will bind to its code, so the code is immutable once created and only a
platform admin can mint one. A clinic that has not chosen a domain keeps
behaving exactly as it does today.
Assignment reuses PATCH /api/v1/clinic/{uuid} rather than adding a second
endpoint. An unknown domain uuid is rejected instead of silently dropped,
because a lost selection would only surface at the first protocol-driven
booking.
Also corrects ADR-0003: resource occupancy does not in fact guard the panel
booking path, which writes appointments.resource_id and no occupancy row at
all, so the doctor slot key cannot simply be dropped.

buildAvailableContexts used ClinicRepository::findByUser(), which is a
findOneBy — so a user who owns two clinics only ever saw the first one.
switchContext validates its input against that same list, so the second
clinic could not be selected at all.
Before tenant isolation this was merely annoying. Since phase 4 it is a
blocker: an environment that cannot be selected is an environment
TenantFilter hides from its own owner. Found by running the suite against
an imported production database, where one account owns two clinics and
its second clinic had become unreachable.
findByUser() stays for the fallbacks that only need "some clinic"; the
context list now uses findAllByUser(). The other 20 findByUser() call sites
are single-clinic fallbacks used when no context is chosen, and keep their
current behaviour — once the owner can switch, UserActiveContext decides.
Removing the fix turns 3 of the 4 new tests red.

Phase 1 of the tenant-marking series. The "which environment is this user
working in?" decision was reimplemented in six places, each reading
UserActiveContext.db_uuid and then guessing whether the uuid belongs to a
clinic or a doctor. Every copy was a place the roles could silently diverge.
EntityContextResolver already encoded the right precedence (explicit
clinic_uuid > stored active context > role) but only five files used it, and
it did not recognise secretaries at all: canActInClinic accepted admins,
clinic owners and member doctors, so a secretary's active clinic context
always collapsed to unknown. That gap is why SecretaryAccessChecker carried
its own copy of the logic.
- canActInClinic now also accepts an active DoctorSecretary relation, and a
matching canActForDoctor covers the personal-practice branch.
- AppointmentAccessChecker, ClinicDoctorAccessChecker, SecretaryAccessChecker,
PatientRecordScopeResolver, MyAppointmentsController and the secretary
dashboard all resolve through it now.
- PatientRecordScopeResolver keeps only its real responsibility: which
doctors' patients are visible inside the resolved environment.
- The resolver answers "where"; ClinicDoctorPermissionChecker and
SecretaryPermissionChecker still answer "what may you do".
Left deliberately untouched, with the reason recorded at each site:
SubscriptionController, InventoryController and TenantTagController check
ROLE_DOCTOR unconditionally and ignore the active context, so a member doctor
sees personal inventory/tags/subscription even inside a clinic. Switching them
changes what users see, which is a product decision, not a refactor.
AuthController keeps its repository because it writes the active context.
tests/ApiTestCase now seeds the "free" subscription plan. db_test had no such
row, so getEffectivePlan returned null, every hasFeature() was false and 83
tests across Patient, ClinicService, Insurance and Appointment failed with 403.
No schema, route, request, response or error code changed.
Tests: 813 passing (was 730 passing / 83 failing). PHPStan clean on all
changed files.

The clinic-member-doctor permission system (ClinicDoctorPermission) lagged the
secretary system: only 6 resources, enforced in ~6 places, dead toggles
(services.update never checked), and a sidebar showing just appointments+patients.
Bring it to parity so a clinic owner can control exactly what each member doctor
does — while an independent doctor stays completely unrestricted.
Coverage: add insurances, addresses, inventory, tags, staff, discounts, sms to
ClinicDoctorPermission::DEFAULT_PERMISSIONS + DoctorPermissionsModal
(subscription/clinic_doctors stay owner-only by design).
New App\Clinic\Security\ClinicDoctorAccessChecker (parallel to
SecretaryAccessChecker):
- denyUnlessGranted(user, resource, action): 403 only for a clinic-member doctor
in the clinic context; owner/admin/secretary/independent-doctor pass through.
- memberClinicId(user): resolves the member doctor to the CLINIC's tenant so the
role-based controllers (Inventory/Tag/Staff/Discount/Sms) stop showing them
their personal tenant in clinic context.
Enforcement wired into 10 controllers alongside the existing secretary gates:
ClinicService (services), Insurance (insurances), Patient (patients+payments),
Staff, Discount, Inventory, Tag, SmsWallet, Payment, PaymentMethod.
Frontend: the guest-doctor sidebar branch now exposes every permitted resource
(gated by can()) plus a «تنظیمات» entry; both settings navs (PurchaseSubscription
Sidebar + SETTINGS_MENU) are now permission-filtered for a scope=clinic doctor,
not just secretaries; my-payments route gets the missing payments permission.
CRUD-button gating already applies (usePermissions is role-agnostic).
Tests: ClinicDoctorPermissionEnforcementTest (member denied/allowed +
independent-doctor-unrestricted); guest-doctor sidebar gating. Backend 375 pass,
frontend 503 pass. docs/api/clinic.md updated with the full resource set +
enforcement notes.

Three reported secretary-access bugs.
1) Settings menu structure. Phase B flat-listed staff/discounts/sms/tags/
appointment_settings/clinic_doctors in the secretary's main sidebar. Mirror
the doctor/clinic layout instead: only inventory + services stay in the main
«مدیریت» nav; the rest live under a single «تنظیمات» entry
(→ /admin/account-settings). Made both settings navs permission-aware for
secretaries: SETTINGS_MENU (menuForRole now takes `can`) and
PurchaseSubscriptionSidebar filter by a per-item `perm`/`alwaysOpen` instead
of role only, so a secretary sees exactly their permitted settings pages and
owner-only items (subscription, secretary-management) stay hidden.
2) Clinic secretary appointment timeline. AppointmentsPage treated a
clinic-scoped secretary as a single-doctor profile: the doctor list was
fetched/shown only for isClinic/isAdmin, so no doctor tabs, timeline, or
booking. Now a clinic-scoped secretary is multi-doctor: fetches the doctor
list, shows tabs, auto-selects the first doctor. The list comes from a new
authenticated endpoint GET /api/v1/my/clinic-doctors returning only the
secretary's ASSIGNED doctors — /clinic/doctor-list is on the public (no-JWT)
firewall and cannot scope by user, so it would have leaked unbookable doctors.
3) Patient record delete. The `patients.delete` toggle was dead: every record
delete (note/medical-record/attachment/call/message) was gated as
`patients.update`. Mapped them to `patients.delete` so the toggle is honored
and delete is controllable separately from edit.
New SecretaryAccessChecker::assignedClinicDoctorIds. Tests: doctor-list scoping,
patients.delete separation (denied/allowed). docs/api secretary.md +
appointment.md updated. Backend 286 + frontend 25 pass.

Extends the secretary permission system to five previously owner-only modules,
so a clinic/doctor can delegate each page to a secretary. All were unreachable
by secretaries before (role-based tenant resolution returned "unknown" → 403).
New permission resources (default-deny, three-place add: entity default,
SecretaryPermissions type, both MySecretariesPage + admin SecretariesPage):
staff, discounts, sms, appointment_settings (view/update only), clinic_doctors
(clinic-only — hidden from independent doctors via `clinicOnly` section filter).
Backend enforcement (SecretaryAccessChecker, three new reusable helpers):
- resolveOwnerEntity(): owner pair from active context — used by StaffController,
DiscountController, SmsWalletController (now secretary-aware resolveEntity).
- canForDoctor(): per-doctor-scoped check (assigned doctor + toggle) — wired into
AppointmentSettingsController::denyDoctorAccess.
- canForClinic(): clinic-scoped check — wired into ClinicController::detachDoctor,
ClinicDoctorPermissionController (view/update), ClinicInvitationController
(create/view/update/delete). clinic_doctors is clinic-context only.
Guards run ahead of any subscription gate; non-secretary roles pass unchanged.
Frontend:
- RoleRoute: staff, discounts, sms-wallet, appointment-settings (doctor+clinic
variants), settings/clinic-doctors routes accept secretary + permission gate.
- Sidebar (secretary branch): five new items gated by can(); appointment_settings
route follows active scope; clinic_doctors only in clinic scope.
Tests: SecretaryResourceEnforcementTest — denied-by-default + allowed-when-granted
for all five (18 total). Sidebar.test — B-resource gating + clinic_doctors scope
rule. docs/api/secretary.md resource list, enforcement map, JSON example updated.

The public doctor list had no location field, so multi-domain consumers
could not tell which city domain owns a doctor. nobat724_front's sitemap
worked around this by fetching the list once per city (35 sweeps) and
subtracting, costing ~13s to build the root sitemap.
Location is resolved in bulk by DoctorRepository::findLocationsByDoctors
using the same rule the city_id/state_id filter applies: the doctor's own
address first, falling back to the address of a clinic they belong to.
Without the clinic fallback a doctor could match city_id=X yet report no
city, which would break the sitemap's per-domain partitioning.
city/state are arrays with at most one entry, matching the shape already
used by the doctor detail response and the clinic list. A doctor with no
address reports [] rather than null. Multi-location doctors get a single
primary city, mirroring the canonical rule on the public site.
Also surface the applied page size as meta.limit. Repositories silently
clamp limit to 50, which previously made clients believe pagination had
ended early — this is what truncated the sitemap to 50 doctors.
The clinic doctor-list endpoint gets the same location data so both
endpoints agree.
Location resolution costs at most 2 queries regardless of page size,
asserted directly against the repository rather than through the
endpoint, since the endpoint carries a pre-existing specialties N+1 in
findWithFilters that is unrelated to this change.

The public doctor payload built `active`/`free_turn`/`hours_of_work` from the
personal schedule alone, so a doctor bookable only at a clinic was reported as
"نوبتدهی غیرفعال". Aggregate over every schedule instead: any schedule with
online booking on and an active day makes the doctor bookable, and the disabled
label only appears when all of them are off.
Three admin-panel fixes for the same class of bug:
- AppointmentsPage took the selected doctor from `dbUuid`, which is the clinic's
uuid inside a clinic context — the slots request 404'd. Use `doctorUuid`.
- TurnsTimeline rendered any error or unknown empty_reason as "این روز شیفت کاری
ندارد". Errors now surface as errors and unknown reasons get a neutral message;
the day-off wording is reserved for an explicit day_off from the backend.
- Admins have no clinic context, so slots fell back to the personal schedule.
They now pick a location from `appointment-booking-locations` and that choice
drives the slot, service and create-appointment requests.
Adds `app:schedule:normalize-format` for legacy rows stored as a bare JSON list
covering only Saturday, which read as day-off for the rest of the week.

Eight required (nullable:false) FKs had no referential action. Set per the
codebase's existing pattern: CASCADE for owned relations (doctor_insurances,
doctor_secretaries), RESTRICT for owner/reference FKs (Doctor/Clinic.user_id,
invited_by_id, subscription plan/period). Only the two CASCADE FKs need DDL —
RESTRICT is the MySQL default.
On prod (opcache.preload + prod container), getRepository(Entity::class)
returned Doctrine's default repository instead of the custom one when the
entity's #[ORM\Entity] had no repositoryClass — so custom finders like
DoctorSecretaryRepository::findAllActiveBySecretary threw BadMethodCallException,
making /oauth/userinfo return 500 after login. Declare repositoryClass explicitly
on all 25 affected entities.
Also add app:create-admin command (create/promote a ROLE_ADMIN user by mobile).
